Claim for Family Assistance
This claim form can be used to claim Paid Parental Leave or Family Assistance (including Family Tax Benefit for the current financial year, Baby Bonus and Maternity Immunisation Allowance).
You do not need to complete this form if:
- you are claiming online; or
- you are already receiving Family Tax Benefit and are claiming for a new born child.
Claim for Bereavement Payment
This form is used if you are claiming Baby Bonus, Maternity Immunisation Allowance and Family Tax Benefit for a child who has recently died.
Lump Sum Claims
If you are seeking payment of Family Tax Benefit for more than one financial year, you will need to complete and lodge a separate claim for each financial year.
- Claim for an annual lump sum payment of Family Tax Benefit for the 2010-11 financial year
- Claim for an annual lump sum payment of Family Tax Benefit for the 2009-10 financial year
