This booklet contains important information if you are claiming by lump sum (see * below) and you must read this before lodging your lump sum claim. You will also be asked to declare that you are aware of the Privacy notice contained within the Information booklet in the Customer's Declaration section of your lump sum claim for Family Tax Benefit.

Claim for Family Assistance
Use this form if:
- you are not claiming online; and
- you are not claiming for a new born child; or
- you are claiming for a first child; or
- you are claiming for an adopted child.
Don't use this form if:
- you are claiming online; or
- you are claiming for a new born child; and
- you are already receiving Family Tax Benefit for another child.
